We at Washington Civil and Disability Advocate are concerned about access barriers across Washington State. This form is for you to report access barriers and violations of the ADA that you or someone you know encountered.

This reporting form is not an intake for Washington Civil and Disability Advocate. Your submission does not mean we will automatically take on this case and do not establish a legal attorney-client relationship. We just appreciate the information you can provide us. If you have an urgent need for assistance, please call (206) 428-3558 for an intake interview or find an attorney in your area to represent you. This is especially true for issues involving employment or encounters with staff, which change quickly and have strict statute of limitations guidelines.
